It would be really good if I could create my own category as I have several hundred items in the hosts file on the pc's I manage and it is a nightmare from an admin point of view. 1 centralised list would be amazing so I could do away with all this hosts file management nonsense

Do you not understand he's asking for greater blacklisting breadth and meta control? While I agree hosts should not be used for antimalware, and while I agree it is badly abused, it is appropriate to abandon hosts in favor of server.

This is where the white list and block list should be under.
Well, if I vote for a domain to be in a category that I want to be blocked, openDNS could block these for me. Is that feasible? My guess is that many users would find this as useful as the original idea.
you could use for filter in fire fox but that would be for just one computer. usually some router have that option that might be better and slightly more convenient too use.

Do you understand how dns works?
What you are asking for is either for OpenDNS to maintain thousands of additional categories - one for each custom list each member might configure - or some sort of local dns check.
The former is not practical unless one radically changes how dns works, the latter is pointless to tie to OpenDns - just use your hosts file, or use a router/firewall that can be configured with a Deny list.
