Implement a system to add a logical link to the blocked domain guide page. Example, administrator blocks hotmail.com either by choosing Webmail category, or blocks the domain individually manually. User attempts to access hotmail.com gets blocked domain guide page. They see the custom message, then two links; Questions? and Not an adult site?, respectively.
I feel it would be logical to auto-change the second link, based on blocked reason, was it a category block, or individual block. After this is determined, the link could change to something as simple as "Flag site for review."
The other part of this is that the site could be flagged for review by the community, or by that individual administrator who blocked the domain individually.

Well it looks like OpenDNS may have implemented this. Using "Not properly categorized?" instead of "Not an adult site?". Looks good.
